JOB SUMMARY:

Assists patients in maintaining and improving their ability to manage their everyday physical, mental, and psychosocial wellbeing within the established Plan of Care. Plans, organizes, develops and coordinates recreational activities in accordance with the patient’s assessment and care plan as well as federal, state and local standards. Safely provides transportation to the patients of FCHS.  Facilitates community Meals on Wheels program. 

QUALIFICATIONS:

Education and/or Experience

  • Previous healthcare experience preferred
  • Previous social work experience preferred
  • Previous experience in social or recreational programs preferred
  • Satisfactory motor vehicle record
  • Ability to operate a motor vehicle in a safe manor
  • Good customer service skills

Certificates, Licensure, Registrations

  • Licensed Social Worker preferred
  • Social Service Designee, State certification or equivalent qualifications
  • Certification as an Activities Professional by an accredited body preferred
  • Valid Driver’s License

  • Ability to calculate figures, add, subtract, multiply, and divide

  • Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form
  • Ability to write reports and correspondence
  • Ability to effectively present information to patients, families, employees, health care providers, etc…
  • Ability to work cooperatively and communicate effectively to maintain good working relationships
  • Ability to work with skill in identifying problems; making frequent decisions regarding method of performance

SECURITY/ACCESS:

  • Will have access to the facility 24 hours a day.
  • Will have access to confidential information abiding by the organizations privacy policies and regulations concerning this information
